White noise on outputs of Blu-100

I am running a Blu-100 (LA v6.00 firmware) with a Blu-BOB, two Blu-10s and an iPad for control. The other day, shortly after triggering an input switch (using a logic source selector to fade down the system, trigger a change to a different source using a parameter preset, then fade the system back up), the system began to produce full volume white noise. There is no noise generator in the system or any other logical source of noise. The noise was definitely heard in the outputs from the Blu-BOB and possibly from the output cards on the Blu-100. To stop the noise, the amps were turned off. Later, the amps were turned back on and the system functioned properly. The system log reporting several keep alive failures (presumably The iPad), but nothing else telling.

The iPad was not used for the next few days (as it was the most recent change to the system) and the system functioned normally the next day. The second day, while changing sources (using a Blu-10) the system produced full volume white noise for a split second, and then functioned normally.

I am just curious if anyone has experienced anything like this before, or has any insight.
