Ui16-fx2 delay volume spike

New board ui16, every time you add any fx2 delay the volume spikes and reds lines the fix channel and the mains : No sound after that : you have to power cycle the mixer to get it working again . Muting does nothing . All other effects work fine : just can’t touch fx2 at all . I have done a factory reset and still same problem. Any ideas Before I sen back for another mixer ?
